Information about the Privacy of Your Medical Information
Hawaii Health Partners is a physician-led accountable care organization with the goal of providing high quality care, increased efficiency, and optimal patient health. Hawaii Health Partners is not itself a health care provider, but rather provides support to health care providers that participate in Hawaii Health Partners’ accountable care network.
Health care providers that participate in Hawaii Health Partners’ network will share patients’ medical information between them and with Hawaii Health Partners for joint health care operations, such as activities to improve the quality of care, increase efficiency, and reduce costs.
Participating health care providers maintain their own notices of privacy practices, which address how each health care provider will use and disclosure its patients’ medical information and patient privacy rights. You care review other health care providers’ notices of privacy practices by visiting their websites or requesting a copy from them.
Hawaii Health Partners is a “business associate” under the federal medical privacy law known as HIPAA. Hawaii Health Partners will only use and disclose your medical information as permitted or required by HIPAA and as permitted or required by a privacy and security agreement that it has in place with all health care providers that participate in the network.
